We have a dedicated member of staff for our Vulnerable patients, Housebound, Learning Disability, Carers, Dementia and others.

This is not for medical conditions but if you are unsure of something or would like to speak to her please call and make an appointment.  They work closely with our Social Prescriber (Please note this is not for complaints).
