Online ‘Pre-Registration’ With The Practice

If you wish to pre-register click on the link below to open the form.  When you have completed all of the details, click on the “Send” button to mail your form to us.

You will then need to visit our reception desk to sign the form and show us a proof of address.  This is often a utility bill or similar document.  It can be a paper document or an electronic copy, if you can show it to us on your phone.  It needs to be dated within the last 3 months.

We will not be able to register you until you have come in to reception.  If you are unable to provide a proof of address, please speak to our reception team who will be able to assist you further.

Pre Registration Form

When you come in to sign the form you will also be given the other forms we use in our registration pack. This is because it can take a considerable time for us to receive your medical records.  It also allows you to register for online access.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in. Please bring your proof of address.

New Patient Registration

Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode.

You will also need to complete a registration form (GMS1) and health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Medical treatment is available from the date of registration.  Please contact reception for further information.

Anyone in England can register with a GP surgery. It’s free to register.  You do not need proof of address or immigration status, ID or an NHS number. You might be able to register with a GP surgery that’s not in the area you live. GP surgeries are usually the first contact if you have a health problem. They can treat many conditions and give health advice. They can also refer you to other NHS services.  For more information click on the link below.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
