New Patient Registration
Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of address such as a recent utility bill (dated within the last 3 months) and proof of ID such as a driving license or passport.
You will also need to complete a registration form (GMS1) and health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team. Medical treatment is available from the date of registration. Please contact reception for further information.
Practice Boundary
Every practice has a boundary or catchment area and patients living within this area can register at the surgery. The practice has recently introduced an outer boundary for existing patients, whereby existing patients who move slightly outside the practice area can remain registered at the surgery. Please ask at reception for details.
